To apply for an e+ card you will need to provide two documentary proofs.
One for your current address:
Bank / building society statement or passbook; utility bill: gas, electricity, water (less than two months old); council tax bill; rent payment book; tenancy agreement; NHS medical card; driving licence, optical / medical prescription, payslip, TV licence
Another for your identity and signature:
Bank, credit or store card; driving licence; passport; EU National or HM Forces ID card
To access some services additional proof is rquired. At the end of the application process we will tell you exactly which documents you need to show us.
