To access some services additional proof is required. At the end of the application process we will tell you exactly which documents you need to show us.

Check which additional proofs you may need to show us.

Applicants under 16
Applicants who are under 16 must supply details of a guarantor for their application.

The guarantor must be a parent, grandparent or guardian and must confirm that they accept the terms of the application and any terms and conditions of the e+ service on behalf of the young applicant.

Receive discounts in local shops - see the e+ discount directory for full details!

All e+ cardholders can access discounts simply by showing their e+ card before making a transaction at the companies who are currently in the scheme. There is no joining requirement, just possession of an e+ card.

Everyone Active Leisure Membership
By adding leisure to your e+ application, your details will be passed to the leisure sites participating in the Everyone Active leisure scheme

Library Membership
By adding library membership to your e+ application, your account will be joined to the system across all library sites in Bracknell Forest Council.

Please note: by submitting this form, you are agreeing to abide by the Library Rules, Regulations, Byelaws and Policies. For more information refer to http://www.bracknell-forest.gov.uk/libraries-public-byelaws.pdf

For more information visit your local library or go online at: Bracknell Forest Libraries

Organ Donor
Sign up to become an organ donor with NHS Blood & Transplant.

Bracknell Forest now offers you the choice of registering with the National Organ Donor register through the e+ card.

If you choose to join, you can select to offer all or some of you organs and the register will be sent your contact details and current preferences. These can be amended at any time. A Donor register logo will appear on your card if you select to join.

Please note, the register exists to record your wishes. In the event that the decision has to be made, your family would be consulted, so you should discuss this with your next of kin

Proof of Age
The e+ card can be used as proof of age. Anyone who verifies their date of birth will have this encoded securely in the card’s chip. Certain services, such as concessionary bus passes for the over 60s, require date of birth verification.

  • Under age 12 the date of birth is printed on the e+ card.
  • From age 12-25 the date of birth and a PASS hologram are printed.
  • Over 25 the date of birth is encoded into the card but not printed.

Except for religious or health reasons, no hats, headgear or dark glasses can be worn for the photograph on a proof of age card.

PASS applicants who do not have an official document bearing their photograph and date of birth will need to get a Verifier to verify their age and identity using PASS Verifier Form A or PASS Verifier Form B. In most cases completing Verifier form A will be the simplest method to use.

Proof of Residence
Use the card to prove your residence in Bracknell Forest. If you are a Bracknell Forest resident and your proof of address shows this, your card can be printed with the term ‘resident’ on it. This makes it easy for service staff to know if you are entitled to resident’s benefits.

Recycling Rewards
Enable customers to obtain rewards through good recycling practices.
For more information on the scheme please visit the Recycling Rewards information page.

Travel
An e+ card showing the English National Concessionary Travel Scheme entitlement is available to Bracknell Forest residents who can prove they are over 60, or fall into one of the eligible disabled categories. This bus pass enables the holder to free off peak local bus travel throughout England.

Please note: Residents over the age of 65 also have the option to trade their concessionary bus pass for a railcard concession instead. Please see the concessionary travel pages for more details.

Please note: This authority is under a duty to protect the public funds it administers, and to this end may use the information you have provided on this form for the prevention and detection of fraud.
